Carmen is Clinical Lead for Long term conditions, specialising in cardio-renal-metabolic conditions such as Type 2 diabetes; Carmen mentors the clinical staff in the practice around any chronic long term condition, running specific clinics for patients referred by other members of the team and also deals with the internal Quality Improvement of these conditions for the practice. Carmen is also a Diabetes educator for health care professionals in the local Derbyshire teaching hub.

Our team of secretaries and administrative workers are here to guide you through the practice by arranging appointments, dealing with enquiries, prescription and results administration and processing referrals.
Receptionists provide an important link for patients with the practice and are your initial contact point for general enquiries. They can provide basic information on services and results and direct you to the right person depending on your health issue or query.

Receptionists make most of the patient appointments with the GPs and nurses. They also perform other important tasks such as issuing repeat prescriptions and dealing with prescription enquiries, dealing with financial claims, dealing with patient records and carrying out searches and practice audits.

Our teams of receptionists also answer the practice telephones and have been trained to help direct your care or enquiry to the right person. Please assist them in helping you whenever possible.

External Teams

The Community Midwife is available to provide help and advice throughout pregnancy. She sees all pregnant mums regularly during their pregnancies, both at antenatal clinics and also at Parentcraft courses; she will subsequently follow up after birth with visits at home.

Trying for Pregnancy? – Our midwife is available for counselling advice.
The District Nurses are highly qualified nurses who have undergone further extensive training to enable them to specialise in providing high quality care to patients in the Community. Home visits are made at the discretion of the District Nurse and are usually only made to patients who are unable to get out of the house, or have been assessed by the District Nurse as having a physical and/or medical problem which prevents them from attending the surgery. The District Nurses also hold wound clinics and ‘doppler’ clinics.
We have a Community Matron in post with the surgery at present. The Matron ‘case manages’ some of our patients with specific health problems, helping to set up packages of care and to help patients avoid upsetting and sometimes unnecessary admissions to hospital. The Community Matron will visit patients in their own home where appropriate.

We have direct access to appointments with physiotherapists.
